Report Canadian T5/T3 in US 1040

Report Canadian T5/T3 in US 1040

This is my first US resident tax return. I'm a bit confused on how to report my Canadian investment income and haven't found a precise answer regarding this. What is the mapping from a T5/T3 to a 1099 so I can report this properly?

A map would be nice. Let us know when you come up with one.
After 20 years, I am severely cutting back on responses. Do not ask specifically for my help. There are a few others on this board that can answer most questions. All the best

I did some research and found someone was talking about this topic before.

Box 11 on the T-5 is your taxable dividends (box 1a on 1099-DIV).

But on my T5, I have Box 24, 25 and 26, my Box 10, 11 and 12 are empty, since box 11 corresponds to 25, I assume 25 should be box 1a on 1099.

@cctax, have you got answers somewhere?
