You should go to their company website and search for W-9. They may have it as part of their own Form numbering scheme.

Thanks. I can't think of who else could pay us besides banks. Home insurance companies? The company that holds our term life insurance policy?

I'll look around on the company websites, and I also just left a message with my CFP at RBC.

As regards your remarks about the W-9:

-- Does that mean it's my responsibility, not the financial institution's nor the IRS's, to initiate the W-9 submission process?

-- Does that apply to every account on the FBAR and to every account and every non-account asset on the 8938?

If so, then why don't we see threats on 1040 schedule B about the W-9s like we do about the FBAR?

The W-9 is the institutes responsibility. Whout W-9, if they "know" you are a US taxpayer, they are obliged to withhold IRS tax on all income paid to you.

Thank you. That clarifies things.

At a local branch of one of Canada's largest banks, the manager and one of the long-term high-ranking employees both know I am a USC. I bank in person, not online, and I talk to these people from time to time. The bank has never asked me to fill out a W-9. I know I'm not asking a question here -- it's just that I find this situation surprising.
