US tax resident how to file Canadian T5 form

I left Canada in December 2008. I transferred my tax home to US by amending my 2008 tax as departure tax. CRA sent me back my revised assessment as departure tax. However, this year I received t5 forms from my brokerage account in Canada for my dividend income . I noticed the brokerage account my departure in 2009. So the address on the slip is my US address. Do I include the dividend income in my US tax return or I need to file return with CRA? I just filed my US tax return a few days ago. Can I amend it without any penalty?

Since you are no longer Cdn resident, this isncome is not taxable in canada. You report it in US.
After 20 years, I am severely cutting back on responses. Do not ask specifically for my help. There are a few others on this board that can answer most questions. All the best
