3520A Part iii balance sheet for the 1st year filing

TFSA was opened in 2009 with TDW self direct, all stocks, no mutual fund. Each year has dividends and minor interests. Need to catch up on the missing reportings and disclosures, starting with 2014. I have read thru most of the articles, some were great advises and experience shared, some were confusing. Need some help here for the 1st year 3520A TFSA reporting (for 2014),

- in PIII balance sheet "Begining of Tax Year" column, should I put 2013 year end figures or all zeros ? All zeros doesn't sound right.
- No withdraw in 2014. In the "End of Tax Year" column the figures should be the book value, not the market value, correct ?
- Stocks were sold in 2016 first time, so 2016 has capital gain/loss but not 2014 or 2015, correct ?
