401K withdrawl as Canadian Citizen/Resident

401K withdrawl as Canadian Citizen/Resident

Dear Experts,

I have a rather unique situation.
Canadian. Worked in the USA.
Put money into IRA and 401K while there.

Brother (who became US citizen) sponsored me for a green card.

Returned to Canada a number of years ago.
No US income, so have not filed US taxes since leaving.

In 2014, finally attained green card while still in Canada.

Planned to return to USA. Then mother got sick.
Remained in Canada to take care of her. Got extension
of green card, Reentry permit.

In 2016, while still in Canada, withdrew money from US IRA and 401K.
Still in Canada in 2017.

I'm now attempting to do taxes with the free version of Taxact
(the customer service is sooo much better than TurboTax).
It's pretty confusing.

To date, I've always used a US address for taxes and 401K accounts.
Mine while I was there. My brother's after I returned.
Keep it simple.

Some initial questions.

So far, Taxact says that I am a Non Resident Alien. Correct?

Should I use my Canadian, or my brother's US address?
If I use the Canadian address, what are the implications?
Will I need to file state taxes anyway?

What are implications for Healthcare?

I'm sure I'll have some more questions as I go.
