Long Form Birth Certificate

Hello All

I applied for Long form Birth certificate online for my kids and what I received is "certified photo copy" of Statement of Live Birth

I have no idea how a long form BC looks like. Is it basically certified photocopy of Statement of Live birth which I filled up at hospital when kids were born

Please let me know if I have correct document or not.

Yes. The long form for Ontario is a certified copy (wth raised stamp) of the original paperwork.

In some other provinced the info has been entered in a database, but not ON>
