I applied for Long form Birth certificate online for my kids and what I received is "certified photo copy" of Statement of Live Birth
I have no idea how a long form BC looks like. Is it basically certified photocopy of Statement of Live birth which I filled up at hospital when kids were born
Please let me know if I have correct document or not.

Yes. This is the Ontario version of the long form.
Some other provinces have entered the statement data into databases, and produce more 'official' looking long birth cetrs. But ontario, at least, still relies on the original statement, and provides a certified (with raised stamp) copy.
