Ex wife refused to cooperate. What to do?

Hi all,

I need an advice. I applied for a permanent resident through in Canada class sponsorship. I have a daughter from my previous marriage who is now living with my ex wife aboard. My daughter will not immigrate here. About a month after I submitted my application, the CIC sent a letter to my ex wife asking to complete some forms. My ex refused to complete the forms and asked me not to contact her again. Do you have any suggestion on how to deal with this problem? I have tried everyway but my ex will definitely not complete the forms. Will her refusal to cooperate affect my PR application? Thank you for your help. BTW I divorced my ex 5 years ago (divorce certificate submitted along with my application). My daughter is 7 years old and has changed her last name back to my ex wife last name since the day we divorced.
