IRS treatment of Canadian Insurance Seg fund

My wife, a US citizen living in Canada has over $100k in a Canadian Sun Life seg fund. There are a few hundred dollars of capital gains and a few hundred dollars of dividends listed on the T3 that they issued. We have been told that they are not PFICs.

How does she declare this income and this fund to the IRS?

Are the capital gains declared on 8949? If so, how? There is of, course, no 1099-B forms and we have no details regarding the proceeds and cost basis.

Is this fund declared on form 8938? Is it declared on a 3520?
