Additional form requested when closing a TFSA?

Additional form requested when closing a TFSA?

I closed my TFSA last year and have completed the 3520 for it.
However I thought I saw a posting on here a few weeks ago mentioning there is an additional form to file when the TFSA has been closed.
Is that correct?
