3520A - Authorization of Agent - Authorizing myself as US Agent?

I am from Canada living in USon TN and been filing US taxes. I am trying to fill 3520A form myself as Canadian institutions wouldn't file it. I want to know if I can authorize myself as the US agent? Instructions of 3520A form has the template on page 5 of instructions of form 3520a, I am wondering if I can fill it by myself in both fields and authorize myself? Thanks!
