1040 X and 540 X

1040 X and 540 X

Now I am prepareing the back filing 8891. Still have two questions.

1) Since there are no dollar amount change in 1040X, may I use the current version 1040X (2010 version) forms for all the previous years?

2) For CA, since I owe tax for the capital gain and interest, may I put the subtotal in the Schedule CA, Others Addition? Or I must seperate it into Capital Gain and Taxable Interest Income lines?

3) I have some capital loss in some year, does it subject to $3000 loss yearly limitation? And need I include the CA schedule D in the 540X?
