Reporting T5 on US Tax Return

My husband (canadian citizen, alien resident in USA) has had some Canadian bonds in his name for over 20 years.
This year his family cashed in the bonds. I have received a T5 statement that shows interest earned in 2009 but no statement of the sale. Do I report the interest earned last year only on our US return, or do I need to find out the original purchase and final sale of the bonds?
If I report 2009 interest only, where do I file a T5 statement on a 1040 return?
