Claiming Grp Ins premiums on Quebec and Federal tax return?

Im a bit confused here. I have about 2300 in Medical expenses i would like to claim including Private plan premiums my wife pays every 2 weeks. However in Quebec on my wifes Box J on her Releve-1 the employer paid 2,011 in premiums while my wife paid the same out of her pocket in her paychecks.

Apparently since Quebec goes by family income and we make to much, i can't use the credit? However for the federal can i claim it? If i can claim it do i use 2300 or the 3%? Im guessing 3% since its a lower amount?

Not a cross-border issue.
