OMERS PENSION

I received a lump sum excess reimbursement from in 2008 of $3216.72
This was a plan from my previous employer which i left in 2007.
I am still in my 30's so i am not receiving a monthly pension.

When i looked 1040 at 16a., do i list the $3216.72 and add 11116 form

do u attach proof of the 3216.72

my T4A received from OMERS for that payment is saying i am a Canadian resident and deducted the canadian tax's

confused?

what happens if the income is not reported can u do a correction?
