What exactly does your agent have to send in every month? Do they send in the remmitance voucher with payment, or a NR-4 slip with payment?

I'm pretty sure it is a remittance slip.

The NR-4 is something YOU get from your agent, at year end, showing what was withheld and sent to CRA.
