Background: Dual CDN/US residents living in the US and starting RRIF periodic payments next year.
Questions:
1. Are both Forms NR301 (for the payor) and NR5 (for CRA) required in order to obtain the tax treaty reduction in withholding from 25% to 15% ?
2. On the US side, is Form 8891 Worksheet still required to be attached to the 1040 return? This Form looks old - I cannot find a new version. Assuming not needed, is there some other specified way to report non-taxable contributions and realized gains?
3. Any other CRA or IRS Forms needed for this situation? (aside from obvious IRS 8938 and FBAR)

1. No. Only NR301 is needed to get the 15% rate, by submitting it to the payor. Sometimes the payor doesn't even need this if they have sufficient evidence that you are a US resident. They should know whether to withhold 25% or 15% based on whether the withdrawal qualifies as periodic.
NR5 would be filed if you believe, that for the current and future tax year(s), you will qualify for even less that the 15% rate, by filing a 217 return. It is never required. If you do file NR5, you must then file a 217 election return for each year in question.
