What form is used to permanently return a green card to USCIS

I-407 is the form. But you can no longer simply send this form, you must appear at a consulate to do this.

Before you give up US citizenship, determine whether the followig expat rules apply to you and if so, the cost.
IRS tax responsibility of expatriates under Code Sec. 877A applies to individuals who on or after 6/17/2008 relinquish U.S. citizenship or cease to be lawful U.S. permanent residents. Generally, Code Sec. 877A imposes mark-to-market regime on covered individuals who are deemed to have sold all property day before expatriation date for its FMV with exception of deferred compensation items, tax deferred accounts, and interest in nongrantor trust of which covered individual was beneficiary day before expatriation date.

IRS form 8854 would be the starting point for such a determination.
