How to get tax refund for the bonus?

I had worked for a company in US as a TN worker, and then went back to Canada in 2010. I have lived in Canada since then. In 2011, the company gave me a small amount of bonus (less than $2000) with deductions made for income, social security, and medicare taxes. Can I claim back the withheld taxes? Is 1040NR the proper form to file?

1050NR will get your fed taxes back. state return for state taxes. Fica is non-refundable.

Remember that you must include the bonus on your Cdn return for 2011, and use the fica as a tax credit.

Since I was not in US in 2011, do I still need to enter my occupation in 1040NR?
