how many pages for TD -F form?

one more stupid question,

I only have one account need to report. When I filled out TD F 90-22.1 form, the only account I have is on first page, and the rest pages are blank. However, my software shows total 4 pages, put my ID in each page, and showed "page __ of __".

Need I submit the whole 4 pages or only the signed first page with the account info is enough?

I only submit the pages I need. That means multiple copies of some pages, and no copies of page 4.
