While filling USA taxes and showing world income in USA, do we have to submit T4,T5 etc in 1040 paper tax filing.

From Canadian tax to 1040:
(1) Which income is to be reported (a) line 150 (Total income) or (b) line 236 (net income)
(2) Are CPP and EI are to be included in total tax paid?

Because of all the extra paper work 1116 or 2555 and Canadian taxes, I think It would be more advisable to ask them to pay for taxes as this would be a consequence of getting paid for 2008. TN renewal by mail
My TN is expiring mid june this year. Looked into the mail procescing dates and noticed they are approx 3 months back. If I apply now by mail say in a week and don't hear from Vermont before my TN expires in mid June, do I have any bonus days I can work legally waiting for them to reply me back??

I cannot comment on the Quebec insurance program, but Turbo Tax standard Candian edition for Ontraio did not ask any questions about the income made after we left Canada. Got wrong RRSP slips second year in a row from this bank in Canada. Last year they messed up wtih my RRSP and T-5 slips.
