So basically not using form 1040NR forces me into another form 8833?
Is this form mandatory if one decides (in my case) to just file a 1040 for the entire year?

This is a very good question ....and let me ask this from a different perspective,,, I relinquished (not renoucnced) my US citizenship on Dec 12th. So for this tax filing I would RATHER file only one 1040 as opposed to filing both the 1040 and 1040NR. BUT...if i file a 1040 for the entire year...esp...

Is there a requirement to send in a copy of your CRA tax assessment with your yearly 1040?
I have in the past (at the suggestion of Accountants) but, since this is my last tax filing in the United States (as I have relinquished my citizenshp), I am anxious to get this filing out of the way.

Starting in 2014, all Americans who file income tax returns must complete an additional IRS tax form. The new form requires disclosure of a taxpayer’s personal identifying health information in order to determine compliance with the Affordable Care Act’s individual mandate. As confirmed by IRS testimony to the tax-writing House Committee on Ways and Means, “taxpayers will file their tax returns reporting their health insurance coverage, and/or making a paymentâ€.

Just to be the devils advocate. Your accountant should not be doing your US taxes unless they have an EIN or a PTIN #. I would take Nelsona's advice and just file correct amount of 8891's going forward. Now that there is some mis-trust with your accountant..you might want to check on your FBARs and make sure they are tabulated correctly. I do my own for that very reason. I relinquished my US citizenship on December 12th 2012.
My question is regarding the "tax year beginning and ending dates" on page 1 on the filers information on form 8938.
Do I just file 8938 from Jan 01 2012 to Dec 31st 2012, or do I have to file only to Dec 12th 2012?
