Never mind, I know the answer: redo the forms.

Applying for a permanent resident visa in a different name than the one shown on your passport does not work well.

OK, if you want to do this, I suggest applying for the visitor visa first, because it becomes more difficult once you have applied for immigration, buit make sure you disclose everything relevant on your application, or you may be accused of misrepresentation.

The procedure is to write to the visa post and inform them that you are withdrawing your application. If you have paid the Right of Permanent Residence Fee already, it will be refunded.
