Can anyone tell me about a W-8 form ??

When we move to Canada will we have to sign this form, who provides it ( bank ??)

If I transfer my IRA to IRA at another brokerage firm, is this considered a trustee to trustee transfer, the assets are electronically sent to the new firm. Would I receive a 1099r and 5498 ???
