8891 question

I am confused about what to enter on the 8891 under NAME.
I have two RRSP accounts and my wife has two RRSP accounts.
I am filing a joint return - we are on H1B - so my wife has a ITIN.

My question is in regards to who I put in for NAME and IDENTIFYING NUMBER on the 8891 when declaring my wife's ...

Question on 8891 and TDF 90-22.1

My wife had three RRSP accounts
* account1 : locked in rrsp account
* account2 : spousal rrsp (non-locked)
* account3 : non-locked in rrsp account

My broker in Canada suggested that we merge the two non-locked accounts so as to simplify management
