Converting T5 to 1099-DIV
Hi, Despite telling my investments that I am a US person and need a 1099-DIV, I did not receive one - only a T5 (and, of course, they didn't do the withholding tax either, but I'm in contact with the CRA about paying that already). It has values in boxes 24, 25, and 26. How can I convert these value...

I should clarify that the TFSA is from my Canadian income, I'm not adding to it. I only plan on being in the US for the 5 years it will take me to complete my degree.
