I only put the interest from my TFSA on line 16 of my 1040. I assume then I should submit a 1040X with Schedule B for each year listing each...

Thank you - in the end I decided to submit form 8082 as the instructions seem pretty clear. As for my Schedule B question I was able to find the answer in the forums - essentially all interest on all accounts is reported as income - but not on schedule B unless over $1500.
