Do I need to file an 8891 for each and every RRSP that I hold in Canada?

I filed an 8833 as well as a 1040X in 2002. I did not file anything in 1999, 2000, 2001 and 2003. Do I need to file 1040X's for all of these years? Also, I understand that for this year (2004) I need to file the 8891 along with a 1040X after I file my 1040. Is this correct?
