Thanks Nelson, I'm filling out the 8891s for 2004 and 2005. The top right hand corner asks for an Identifying number. The instructions clearly say this should be a SSN or ITIN. The original 1040s (filed jointly) for 2004 and 2005 had my wife's ITIN. We just got our permanent residencies in 2006, so ...

If I am filing 8891s for past years (catching up for past sins), should I be using the 8891 that was provided for that specific year, or can I use the new form that doesn't have a date?

I have a real simple question about the 1040x form. I need to amend a previous year to add a 8891, and NO other changes. Awhile back, in a post started by jstone, nelson suggested that he could just amend his previous year's 1040 (2004) with a form 8891, rather than go back all the way to 2002. Is your advice based on new info?
