Thanks Nelson, I'm filling out the 8891s for 2004 and 2005. The top right hand corner asks for an Identifying number. The instructions clearly say this should be a SSN or ITIN. The original 1040s (filed jointly) for 2004 and 2005 had my wife's ITIN. We just got our permanent residencies in 2006, so ...

I have a real simple question about the 1040x form. I need to amend a previous year to add a 8891, and NO other changes.
