Help with my final 3520/3520A??

Hi there.
I closed my TFSAs in 2014 and need some help on filling in the forms. I filed in 2013 and 2012 so have the basics of it down.
However, how do I bring the balance of the account down to $0 properly. I am assuming i need to report it as a distribution but want to make sure I do it right as it should definitely not be added to my income.
