pro forma 1040 question

I understand you include all world income for your spouse as well. What about spouses unemployment insurance payments?

Yes, all income. You are pretending to live in US. Cdn UI is reportable on 1040,.

1) is the universal child benefit also reportable? I understand there were changes in 2016.

2) if these are reportable in 1040, will it be an issue if we report them in our Canadian returns?

Whatever is reportable on your Cdn return is also reportable on a 1040, whether it be the normal 1040, or the "pro forma" 1040 which is prepared in conjunction the XVV(2) 1040NR.

A US taxpayer living in Canada ends up reporting ALL income on both returns. This is no different.

which lines we can report these on form 1040? line 19 and 21?
